RESPONSIBILITIES
Performs personal care and patient-specific activities that assist the patient with activities of daily living, including but not limited to:
Help with patient's personal hygiene care
Assist with dressing
Prepare meals and snacks according to instructions
Accompany patient on errands or medical appointments
Perform light housekeeping activities
Provide hospice supportive care and medication reminders
Assist with the prescribed range of motion exercises
Assist with blood glucose monitoring
Use lifting equipment such as gait belts and Hoyer lifts
QUALIFICATIONS
CNA license or PCA certification
Ability to treat clients with dignity and respect
Ability to interpret the client’s provided plan of care
Valid driver’s license and transportation
Ability to be flexible and adapt to new situations
Tolerance to small pets (i.
e.
dogs and/or cats)
Must be able to be reached via phone or email for shift schedules, client updates, and/or emergencies
Able to lift a minimum of 25 lbs
Prior experience with dementia patients and senior care, is a plus!
Must be able to pass a background check
