GENERAL SUMMARY
The Care Access Coordinator is responsible for facilitating transitions of care, connecting patients with CHKD and community resources, and supporting timely patient transitions to appropriate levels of care.
Care Access Coordinators operate as part of a comprehensive treatment team to ensure a patient and family have necessary resources when transitioning levels of care.
This position reports to Manager of Acute Mental Health Services and Operations.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Responsibilities:
Coordinates care based upon current discharge plan and recommendations from treatment teams.
Ensure plan of care is established at time of admission and updated plan as needed throughout treatment.
Coordinates with family and providers to assess need, provide resources, and schedule follow-up care.
Provide feedback to treatment team regarding access to resources and potential barriers.
Participate in integrated treatment team meetings
Communicates plan with managed care organization and/or address barriers to access prior to transition.
Collaborates and maintains working relationship with community providers, team members, and families.
Consistency meets documentation standards for quality and timeliness.
Maintains confidentiality of patients/families medical information in accordance with department and hospital policies.
Adheres to established productivity measures.
Performs additional responsibilities as assigned by management.

MINIMUM EDUCATION AND EXPERIENCE REQUIREMENTS
Bachelor’s degree required with a focus in social work, human services, or related field, preferred.
Understanding of current community resources, required.
Demonstrated ability to organize, prioritize, and communicate effectively.
Minimum of 1 year of applicable work experience with pediatric population or families and/or in acute care settings, preferred.
Increased Security Clearance may be required: FBI fingerprinting, criminal background check, and Child Protective Services registry search.
WORKING CONDITIONS
Normal office environment with little exposure to excessive noise, dust, temperature and the like.
